Through working with Stockport Support Hub, Jane was provided with emotional and practical support following a bereavement.

Jane and her husband, Bill, were referred to Stockport Support Hub by a concerned friend who noticed that the couple were struggling to manage at home. Jane was Bill’s primary carer, despite facing her own health challenges. The couple needed assistance with managing day-to-day tasks and navigating the complexities of disability benefit claims. A referral to Adult Social Care had already been made, but further support was necessary.
After an initial conversation with Bill and Jane, the Support Hub made a referral to Carers Support to ensure Jane had access to additional carer-focused services. We also placed them on the waiting list for a dedicated Your Support Key Worker who could assist them in completing the necessary benefit forms to meet their financial needs.
A week later, Jane reached out to the Support Hub again with the sad news that Bill had passed away. Devastated by the loss and overwhelmed by the administrative tasks that had to be completed, Jane was in need of urgent support.
Stockport Support Hub prioritised Jane for the allocation of a Your Support Key Worker. The Hub team provided emotional support and reassurance, reassuring Jane that we would assist her with the practical challenges she faced at this difficult time.
